2005-2006 Grade 10 Citizenship Award Sample Nomination Form

A. SELECTION

A student shall be chosen in each regular secondary school from students presently completing Grade 10, by a school committee consisting of the Branch President (or his/her designate), the Branch Awards Committee Rep (if the Branch has one), and at least one other teacher. The committee may also have student rep(s) if desired.

B. CRITERIA

  1. The nominee is Grade 10 student who contributes to the quality of life in the school unit (not necessarily a leader).
  2. The nominee shows concern and consideration for others without regard for personal recognition.
