Annual Offence Declaration

Regulation 521/01 requires School Boards to collect Criminal Background Checks on all existing and new employees and service providers. The legislation also requires Boards to collect an annual Offence Declaration from all employees and service providers. The intent of the legislation is to ensure that schools and classrooms are safe places for students.

All employees must complete an Annual Offence Declaration by 2006 September 01. This may be done in two ways:

1. Go to the Employee Portal. Under Personal Information - Complete Your Offence Declaration - Just click and you’re done! It’s fast and secure!

OR

Complete the Offence Declaration form and mail it to the attention of John Cuddie - Manager HR Services. Forms are available through the union office and available at the Education Centre and CEC’s reception area.
