BIOLOGICAL SPECIMENS ON DRY ICE

Procedure for Air Waybill. This applies only to Non Dangerous Goods shipments. 
A shipper's declaration is not required for dry ice. Do not identify dry ice as Dangerous goods.
The following statements are required in the Complete Description of Contents box on all air waybills.

For NON INFECTIOUS SPECIMENS On DRY ICE  The Following Statement In The Description Of Contents Is Acceptable.
HUMAN, NON-INFECTIOUS Diagnostic Specimens, Not Restricted. In compliance with IATA pkg. instr.650 
DRY ICE, 9, UN 1845, I I I, ___X___KG, 904, Dangerous Goods Shipper's Declaration Not Required. 
 
For NON INFECTIOUS SPECIMENS not on  DRY ICE  The Following Statement In The Description Of Contents Is Acceptable
HUMAN, NON-INFECTIOUS Diagnostic Specimens, Not Restricted. In compliance with IATA pkg. instr.650
 
The package should be labelled as follows:
  1. Dry Ice ____X____KG. (The first blank is for the number of packages. The second is the weight of dry ice.)
  2. UN #1845 CLASS 9 I I I. 904
  3. The shipper's and recipient's names and addresses must be on the Package.
  4. A CLASS 9 Miscellaneous label must be on the package. (See the example below).
