Delete the word Radioactive and either 'Passenger and Cargo' or 'Cargo Aircraft only" .    Signature of Shipper. 
 
Proper shipping Name.    Amendment or alteration signed by Shipper. 
 
 
( Class 6.2 proper shipping name is INFECTIOUS SUBSTANCE AFFECTING HUMANS).  On the Air Waybill. 
(technical name, Physical state and Risk group may be required).  The Handling Information box shows: 
Class or division.    "Dangerous goods as per attached Shippers Declaration". 
 
 
UN or ID number preceded by prefix.    Cargo Aircraft Only, if applicable. 
 
 
Packing Group. 
Subsidiary Risk. E.G. Dry  Ice
