The Basic Fee
($1742.50) includes:
- Transportation from place of death, record and file legal documents,
transportation to crematorium, CPP forms and Proof of
Death Certificates are completed by our
staff and packaged for mailing. We provide a general executor's
checklist to assist in settling an estate and how to accomplish many of these
tasks ($1215)
- Coroners fee, cremation
fee ($510)
- Provincial levy and Municipal registration
fee ($45.00)
- Temporary container from crematorium for
cremated remains (no charge)

The Memorial,
Gathering or Reception - family on own ($485)
includes:
- a registry and 25 thank you cards ($85)
- Full colour custom designed
memorial cards are available for $2.00 each
- A memorial DVD is available with
25 of your photographs and a song of your choice ($150 extra, $10
each additional copy)
- Use of Mt. Pleasant chapel for an hour ($250)
- clergy honorarium ($150)

Anatomical Donation ($852.50) includes:
- Transportation from place of death, record and file legal documents, coordinate
with department of anatomy staff and transportation to University
of Western Ontario, Department of Anatomy.($810)
- CPP forms and Proof of Death
Certificates are completed by our staff and packaged for
mailing. We provide a general checklist to assist in settling an
estate and how to accomplish many of these tasks.

the cremation is possible at one of the crematoriums. Please call us

| Identification - In
some circumstances you may wish to identify your loved one prior to
cremation. This is
possible at London Cremation Services at a prearranged date and time
prior to cremation (No Charge) |
